Sports Physicals – This service is available all year long instead of just during back to school season. This is ideal for parents of children who participate in school sports or extracurricular activities. Our sports physicals can serve as a medical release for athletic programs or summer camps.

Department of Transportation Physicals – Phoenix Family Medical’s providers are authorized to provide the Medical Examiner’s certificate that clears commercial drivers for their annual physical exam. The exam is valid up to 24 months, but your employer may require exams at a more frequent rate, especially for patients with health risks like high blood pressure, diabetes, high cholesterol, and any other condition that can lead to an unforeseen medical event while on the road.

Allergy Testing and Shots – Allergy testing is often prescribed by a primary care physician or allergist as a specialty service. We can do these same-day tests in-house.
